depends on your escort better check in with them fist but usually you can book multiple services but once you're already in the meeting, I don't think you can still add a service as it would ruin the escort's schedule.

Yes, you pay for every services babe. Also for your first question, better finalize the services that you want and officially book them as chances are, they have other meetings too and they need to arrange their time as well.
